Insurance
One of your steps to surgery, is to meet with our Patient Advocate to discuss your available insurance benefits and payment options. We work with patients to navigate this process.
Our Foothills Weight Loss Surgeons and Fort Sanders Regional Medical Center are in-network providers for most insurance plans. Insurance benefits and criteria for weight loss surgery can vary greatly by plan and by employer, and we will verify the benefits and criteria specific to your plan. Most insurance companies base their coverage criteria on the National Institutes of Health (NIH) guidelines for Body Mass Index or BMI:
- BMI of 40 or greater
- BMI of 35-40 with significant co-morbidity

BMI and co-morbidity requirements apply only if surgery is a covered benefit under your policy. Some policies have exclusions and do not cover bariatric surgery.
Self-pay
Most surgery-related fees are not due until after you are approved by any applicable insurance. This insures that you will be having surgery before you make payments. Also, you can take advantage of the time in which you are preparing for surgery and meeting insurance guidelines to arrange payment or save for the expenses you will incur.
$12,850 Gastric Sleeve Available at Premier Surgery Center
If you are an appropriate candidate, the Sleeve Gastrecomy procedure can be performed at the outpatient Premier Surgery Center on Papermill Drive. The $12,850 price includes the surgeon, facility, anesthesia and BLIS insurance coverage for complications. For an additional $500 expanded BLIS coverage is also available. Dr. Colquitt and his team will determine if you are an appropriate candidate for outpatient Bariatric Surgery.
Blis: Peace of Mind for the Pre-Pay Patient
Foothills Weight Loss Surgeons participate in Blis, an innovative insurance coverage that allows our bariatric pre-pay patients to be covered should a complication occur within certain time frames following surgery. With Blis coverage our patients are not responsible for the costs associated with the care of that complication.
** The complication protection is exclusively part of the Package Price for Self-Pay weight loss surgery. We are happy to provide this benefit to our patients and ease your mind and your wallet in this important decision to have a life-saving surgery.
